Most setup issues are resolved by identifying which layer is refusing the work: the provider connection, the workspace guard, or a deliberate safety policy.
| Symptom | What to check |
|---|---|
| Cannot reach a model | Confirm --base-url, model ID, and provider credential. Without a configured provider Orcacode can use a local model endpoint. |
| Write is refused | Read the current file first, use edit_file for an exact replacement, and confirm the path is inside the workspace. |
| Tool denied in plan mode | This is expected for shell, writes outside docs/plan/, subagents, persistent compute, processes, and MCP. Switch to normal mode after reviewing the plan. |
| MCP server will not connect | Check its executable is on PATH, the whitespace-split command is valid, and its credentials are in the parent environment. |
| Changed AGENTS.md is ignored | Start a new Orcacode process to load edits; /clear starts a fresh conversation but reuses the instructions loaded at startup. |
| Need prior long tool output | Use read_tool_result with the supplied call ID and page offset. |
